Families are encouraged to come in and tour the center prior to enrolling their child to see their child's classroom and meeting the teachers and staff that will be caring for their child.

We encourage family members to call or visit us any time during business hours and to ask any questions.

Forms required for applying:

Additional items that will be needed include:

  • $25 Registration Fee
  • An original Birth Certificate (staff will make a copy at the center and give the original back)
  • A copy of a current physical (within the last 6 months)
  • A copy of their shot records (usually on their physical)
  • At age one, children will need a lead screening (unless other wise stated by the doctor)
  • At age one children need a TB test and age two a Chicken Pox Shot (unless other wise stated by doctor)

Items to bring once your child is enrolled:


  • Enough bottles for the amount of time in care
  • If on special formula or parents prefer a certain formula, that will need to be provided by family
  • Diapers and wipes
  • A couple extra pair of clothing in case of accidents


  • A pillow and Blanket for nap time (a soft toy is fine)
  • Extra clothing in case of accidents  Diapers for those not potty-trained.

Prescription Medication: MCDC will administer prescription medication that is prescribed to your child by their doctor.  You must complete the required form and physically hand the medication, in it's original container, to a staff member.
